WORKER'S PERMIT INFORMATION

After you have been offered a job, follow these steps to get your work permit:

Step 1: Download and print the State of Maine work permit form.

Step 2: Fill out the form.

Step 3: Take a picture or make a copy of your birth certificate

Step 4: Email the completed form and a photo/copy of your birth certificate to Mrs. Picard (mpicard@rsu16.org) in the Main Office. Please do not send a photo of the completed form.  You may also bring these documents to Ms. Picard in the Main Office.
