After you have been offered a job, follow these steps to get your work permit:

Step 1: Download and print the State of Maine work permit form.

Step 2: Fill out the form.

Step 3: Take a picture of your Birth Certificate

Step 4: Send the completed form and a copy of your birth certificate to Mrs. Picard (mpicard@rsu16.org) in the Main Office. Please do not send a photo of the completed form.
