School Registration Forms

The RSU #16 administrative staff are in the process of streamlining the registration forms across the district to have them be consistent. Presently, all forms are different at each school and this can make it difficult for parents/guardians enrolling their children in more than one school, as well as the registration process itself at the district level. Once this process is complete we will post the forms to be downloaded here.  In the meantime, you can find the district registration process here at this link, and that will give you the school contact information and let you know what is needed to register your child.
